Trouble with registration

The registration form is not continuing do to information that is not avalible. Contacts is what I have entered amd nothing else to add.
And one other item
How can we proceed with the enrollment. And whom can i speak with . Please provide the contact information

Would you please verify if you have filled out the mandatory fields of the form? Are you getting any error message? Please share a screenshot of the enrollment page.
