Please need help . my daughter's elementary  admission.. this what's the mail read  

That my daughter application for grade 2  Is REJECTED. Please fill the separate form due to one parent is Catholic and the other one is not.

I had check and checked , can't find the separate form on the account page.

how do I go about this.
