Unable to enter street address

When attempting to fill out the D49 enrollment form I can enter my state but cannot enter my street address (no field to enter).  Yet it will not let me proceed with the enrollment without my street address.

I am sorry you were unable to submit the enrollment form to your school district. PowerSchool designs enrollment forms based on your school's specifications. I request reaching out to the Enrollment Section of your school district as they would be able to assist you with completing this field on the form.
