Open enrollment lottery form

Hello can someone please direct me to where I can find the open enrollment lottery form for San Juan Unified high schools? The website states it’s not available and enrollment opens December 4th. Thank you

We see that you have been working with our PowerSchool Support Team and the resolution was provided to contact the district since the form was not released.
