The Contacts form is not populating properly. Only the 1st contact has fields for First and Last Names. It also will not allow the form to process forward if only 1 Contact is provided.
Same result in both Google Chrome and Microsoft Edge, so this does not appear to be a browser issue.

If the Contact form is not populating information in most browsers, it could be that the form fields have not been updated correctly. I recommend reporting this issue to the district directly and they should be able to work with the Enrollment specialist to resolve it.
