We recommend reaching you to the Technical Contact of your district who should be able to connect with the PowerSchool SIS Support Team. The Support Team should be able to take a closer look at your setup and assist you further with the troubleshooting steps.
It is likely a setting on the report that is causing the issue. If the report is set to current school only, it will not pull in the historical grades associated with another school. That is the first place I would look. Another possibility is if the grades were put in manually because of a transfer from another district, check to see if the individual grade is set to display on transcript.
