Unable to go forward after entering in Contacts. I hit the next button and nothing happens.
Could you please confirm the internet browser you are using to access the Enrollment forms? Our Enrollment forms are designed to work best with Google Chrome or Firefox browsers. I recommend accessing the Enrollment form using one of these browsers and also switching to a laptop/desktop if you are accessing the form on mobile.
