I completed my child's registration, however I had forgotten to do the kindergarten questionnaire. 

Once I filled out the questionnaire, I was unsure of where exactly to upload it under. Which section?

I am having a related problem. I did not see a kindergarten questionnaire. I accidentally pressed submit on my child's application, and no kindergarten questionnaire was requested.

Can you tell me where you found it? Can it be submitted to School Choice in another form?
