Looking for a Consent to Record Form

Good morning,

Our Occupational Therapist requested a Consent to Record that a client signs giving permission to an occupational therapist to record their assessment session whether through video or audio, for the purpose of reviewing and analyzing their performance during the evaluation. 

I do not see such a consent form in Tienet. How do we get one? Can we get one through Powerschool/Tienet?

Someone within your district would need to build one within PowerSchool as an object report for form letter.  Or you could find another district that has one already built for PowerSchool and see if they would be willing to share for importing.  As fare as availability in Tienet, you would need to have someone check with them and their developers to see.  I am not familiar with their systems and don't know if local forms can be added.
