Questions for Parents

You can use this section to ask questions specific to a parent/guardian. These questions will only be asked once, even if the guardian is filling out a form for multiple attendees/children.

 

So, what kind of information might you want to collect from a guardian?

 

Here are some examples:

  • Are you able to volunteer?
  • If you are able to volunteer, what time can you start?
  • Are you able to drive to the event?
  • If you can drive, please upload a copy of your driver's license.

 

 

Questions About Each Child

You can use this section to ask questions specific to each attendee/student. These questions will be asked for each individual attendee/student.

 

So, what kind of information might you want to collect about an attendee/student?

 

Here are some examples:

  • Homeroom Teacher
  • Grade
  • Does the attendee have any allergies?
  • Student Number
  • Date of Birth
  • Skill Level

 

Note:

  • Guardian and Attendee labels can be branded to suit your organization.

 

  1. Click on the “Admin” tab in the top menu (Please note that only administrators of the account will have this function)
  2. Click on the "Customize Role Labels"
  3. Customize the names that will appear on your forms.
    Example) Guardian = Parent, Attendee = Student.
  4. Once you have changed your labels, don’t forget to click the “Save” button.
