Error message for face-to-face return form

This is the link for the form needed for face to face return in January. It keeps popping up an error message not allowing me to complete it.

I'm sorry you are facing difficulties completing the form. I recommend reaching out to your child's school district who should be able to assist you in completing the face-to-face return form.
