Form Not Progressing or Saving

I am attempting to register my child, but form only progresses to "Demographics" section then refuses to save or progress.  The previous sections were completed and saved without issue.  Please advise.

Would you please try clearing the cache and cookies or switching your browser and continuing with filling out the form?
