Getting an error when a teacher tries to enter an incident log. A pink box pops up with "Unsuccessfully to submit incident." Yes, that's the actual error language. Went through this https://docs.powerschool.com/PSHSA/22.9/incident-management/teacher-incident-referrals with no luck.
The only thing I could find that might be related is while working on adding action taken and attendance code there are a couple of the 'map incident log action taken and the attendance code' that are made, but despite clicking on delete, doesn't seem to actually delete.

In case, you are still facing the issue, we recommend reaching out to the Technical Contact of your district who should be able to assist you in restarting the PowerSchool server which might help you resolve the issue.

Went through and everything was set per that. Just in case there was some kind of weird setting that didn't carry I wound up disabled, saved, then re-enabled and saved. Seems to be working for now. Will update if teachers say otherwise.
