Even though I filled in the following field:

Place where your child receives dental care

 

I am still getting a red flag so my form will not send. I have been trying since March 12, 2019 to complete this form!!

 

I'm beyond aggravated and frustrated. I have all the information filled out but the system is not accepting my form!

I have created an account, it says that username is not existing and when I try to create a new account it says I have an account already with that information and it will not allow me to reset my password

Hi! @Megano1018,

 

If you already have an account and do not remember the username, I recommend reviewing the article on Forgot Username that should help you in logging into your account.  I also suggest reviewing the articles listed below to confirm the email address associated with your Enrollment account and resetting your password.
