PowerSchool SIS: STATE DISCIPLINE REPORT FOR SPECIAL EDUCATION STUDENTS

I need help creating a report that will help me complete the State Discipline report for SpEd students for 2021-2022 School Year.  It should include :   Student Id , name, grade, OSS, ISS, Days of Suspension, dates suspended, Infraction description  

 

Any ideas or how I can create this report?  If it exists in the system, how and where?
