LAMBS Temporary Position - Reference received an email that did not apply to them

I am a student who was recently accepted into the LAMBS program, and I needed to add some references. One of the references that I added was listed as a friend, but they received an email with a form with questions as if they were a supervisor or colleague to me. Is there a form they can receive which has questions that apply to them? To clarify, they are an adult friend of my family that has a good understanding of my character.
