A commonly overlooked issue on our forms is the Priority page. Each contact listed on this page needs to be assigned a unique priority number. For example, if the parents are listed as "1" and "2", the emergency contacts on the page will be listed as "3", "4", and so on, without repeating any numbers. Once this is completed, you should be able to successfully submit your form.
