Can't submit form

When i try to submit the form it tells me thati should complete all required fields. I scanned back into the form and  im missing coosing the relationship to the contacts but when i try to edit it i cant write down anything.

I am sorry you were unable to submit the Enrollment form. I recommend reaching out to the Enrollment section of the school district who should be able to assist you with enrolling your child to the school.
