How do you print an NJ A45 form from powerschool?

You should be able to print this report by navigating to the Reports (State Reports) > NJ Student Health History and Appraisal Report from the State Reporting page.
Using the above state report is on a tab that has many other state reports that the health office staff will not run. Do we need to lock down all of the reports on that tab in order for them to run the one report they need? (NJA45) Is it accessible for health staff any where else other than the main state reporting tab?

Currently, the NJ-Student-Health-History-and-Appraisal-Report is currently only available under the State Reports tab.
If you need to have the report available on another page, you can share this idea as an Enhancement Request on PowerSchool Ideas Portal so that the product team can take a look at this. This article should walk you through how to raise an Enhancement Request.
You can work with our Customizations team who may be able to build this custom page for you and add the link to this report for the teachers to access. While this service does have a cost associated, we want to make sure you are aware of all available resources. You can submit a request for customization here using the Customizations form.
However, the only workaround is to lock out the other reports on the tab to run this report only.
Thank you. I am trying the lockout all other reports on that page. I came across an issue where I lock all reports down, but when I get to the final report to give access the reports above unlock too. Strange, but I am working with support to resolve this. Have you seen this happen?

This is unexpected behavior. We recommend that you continue to work with support through the case which has been escalated for further troubleshooting.

When I print the A_45nj, it is missing dtap and hep B vaccine, and the screening dates are not in ascending order? How can I have this fix?

I recommend reviewing this article that will help you get started to make sure that you have this report setup correctly on your end. In order to troubleshoot this further we will have to take a closer look at your setup while accessing your site. In order to protect your data privacy, I recommend working with the technical contacts in your school district who should be able to create a support case on your behalf with the PowerSchool SIS support team.
