In each assessment data file, a Student Identifier column will be included. Please provide the exact name of that column and indicate if the value used in this column is a State Student ID or a Local Student ID (pulled from SIS)?
Please identify one column which represents the Primary Score for each assessment data file.
Indicate the type of assessments you provided. Please select only one type for each assessment.
Please enter the lowest and highest possible scores for this assessment. Leave blank if there are multiple score ranges.
