pre-enrollment form technical issue

There is a technical issue with the pre-enrollment form that just opened today (1/7/25) (https://secure.infosnap.com/family/ActionForms/Public/42650).

The "I am enrolling my child in" question has no drop-down options (it simply says "Select"). Presumably, this is where parents select Early 5s or kindergarten, etc. But because of the technical issue, this isn't possible.

I've contacted technical support but they simply suggested contacting the School District.
