CHDP and dental waiver !?

Does anyone know where I can download or get copies of the CHDP and dental waivers to send my doctor/dentist ? My daughter is starting kindergarten this August and I of course am unable to get a hole of anyone in regards to where I can find these form !? Please help thank you !

I recommend reaching out to the Enrollment section of your school district who should be able to assist you with getting copies of the CHDP and dental waivers to send your doctor/dentist.
