reenrollment form questions

I'm reenrolling my rising 9th grader into Franklin County schools (was a previous student in the 4th grade at Louisburg Elementary) and I have questions about what I need to reenroll. Do I need to submit the North Carolina Health Assessment Transmittal Form, health assessment, and immunization record since she's been a previous student?

The PowerSchool Community is a great resource if you have any changes to be made while completing fields within the enrollment form. Each district, however, has a different registration process and can provide the best steps to move forward when enrolling students.

I suggest you reach out to the district directly to enquire about the required documents as they will be able to assist you with the most updated information.
