Is there a way to unclick the form if the option "No, I do not give permission for the school department to photograph, videotape or audio tape and/or use my child’s name in any print or electronic media, or in any other school-related publicity efforts." was clicked accidentally?

Once the form has been submitted, you cannot change the data in the form. I recommend contacting your child's school district who should be able to correct the necessary information in the form.
