Impossible to final the enrollment form

Good afternoon

I have just filled in all the required information and document for my son but I am not able to finalize it as I still have to check the following bow

"I consent the use of the Medical Information provided here by the school."

the issue that I do not see any bow to check

I am sorry you are experiencing issues accessing the Enrollment form. I recommend reaching out to the Enrollment section of your school district who should be able to assist you in enrolling your child in school.

The school website should contain details to contact the Enrollment section of your school.
