Registration submission error

I submitted a kindergarten application. It was correct before submission. When I open the application to print copies, I see that my name was substituted where my son's name (applicant) should be. It kept his middle name in the correct box but took his name out? Now, I can not retrieve the application as it went through. Can it be retrieved?

Has this occurred before to any other applicants?
