"Please Select unique school values" error

I am getting 4 errors on the Student Residency Questionair page all stating "Please Select unique school values"; however there are only yes or no questions on that page. This is blocking me from registering my child. Please help.

I have cleared the cache and cookies and tired a different browser on 3 different devices. None of these helped. I even tried deleting the original registration form and recreating the entire thing and running into the same issue. I thought this was the platform's support team. Is there another support I should be using?
