Validation errors in Special Programs ran through DEX

I am receiving the same error message everyday despite updating the system with the correct information. I need a step by step when I receive a validation error. I am going to try to include an example. If you will notice the IEP review date says 01/01/0001 despite putting in the information everyday for the last 2 weeks
