Enrollment forms/Google forms question

Has anyone ever used a google form in their enrollment forms before and did they have any issues? I have 2 I want to use, but the links you have to copy and paste in a new browser to get them to open, instead of just being able to click on what I thought was the links in the questions..

I have never used google forms in the enrollment forms but I have had links in the form. Maybe it's the format you're using. Here is what I use:
